RELATIONSHIP QUESTIONNAIRE

This Relationship Questionnaire is for couples that have chosen the Bespoke Wedding ceremony or Commitment ceremony package.  Its purpose is to provide me with information to customise and personalise the ceremony content.

A few tips:

• Each person is to complete the questionnaire.  I find if you complete it in private without the other persons influence then the content is usually more authentic and fruitful

• You do not have to answer every one of the questions

• The more information you provide, the easier it will be to create a completely unique ceremony

• Try to be as open as possible and enjoy the process
